Current Students: Please fill out the transcript request form and give to the registrar in the counseling office.  You must also have a test score release form on file in order for FMHS to send SAT or ACT test scores.  

Graduates Only: Official transcripts can only be mailed to the college or institution and cannot be mailed to the parent or student.
